These
documents should provide the user candidate with:
a. An introduction to use and safety around the HUP5
and 6 systems.
b. Formulation of questions before the on-site introduction
to the systems
c. General knowledge of nomenclature used during orientation
d. General knowledge of locations of the MR Suites
and switches

Application Requirements
a. Submission of the POR Certification obtainable
through the OHR website

After successful completion of the above application
requirements a certificate of Basic User Approval
will be given to the operator.

Operator Monitoring
a. Use of the system by the operator will be monitored
through required documentation at the scanners. This
will be a shared responsibility between Faculty, staff,
and other approved users. All infractions, unsafe
practices, and incidents should be reported to Norman
Butler (215-349-5298) or a CAMRIS member.
b. Infractions will be documented and if a sufficient
number of these occur then upon review by the CAMRIS
committee privileges may be revoked. Some typical
infractions are listed :
aa. Safety violations of any sort
bb. Insufficient documentation of system use (ex:
No explanative entries in log books)
cc. Inappropriate header information entry
dd. Non-compliance to cleaning or preparation procedures
ee. Use during unscheduled hours
